DFPS 2085 B Form: Designated Medical Consenters for STAR Health Members

In an effort to help serve children and youth that enter into foster care and are Superior STAR Health members, it is important to understand the purpose of the Texas Department of Family and Protective Services (DFPS) 2085 B form.
This form allows DFPS staff to designate the Medical Consenter for these members. It provides the important information caregivers require to meet the child’s health-care needs. This form also allows the member to access health-care services immediately upon DFPS removal. This means that when a child is brought into foster care and is need of health-care services, the designated Medical Consenter can take the child to any STAR Health provider or pharmacy. The Medical Consenter can show this form as proof of STAR Health eligibility, before the child is shown as eligible with STAR Health.
Please note:
- The DFPS 2085 B form will authorize placement in a regular kinship placement.
- All STAR Health providers (including pharmacies) are contractually required to provide services when the DFPS 2085 B form is provided.
- If a provider is refusing to see a member with the 2085 B form, please escalate this immediate concern to a Superior STAR Health Member Advocate by calling 1-866-912-6283.
