Sports Physicals as a Value Added Service

As spring training and sports begin, Superior would like to remind you that we cover sports physical as a value added service for our members.

• Superior will reimburse sports physicals for eligible members:
  • STAR, STAR Health and CHIP members only
  • Ages 4-17 (STAR and CHIP) and ages 4-18 (STAR Health)
  • One (1) per calendar year

• Claims filing guidelines:
  • Diagnosis Code: Z02.5
  • CPT Codes: 99382-99385 or 99392-99395

• Reimbursement will be $35.00 (there is no co-pay).

• Please note that a sports physical is NOT the same as a Texas Health Steps checkup.
  • If a child is due for a Texas Health Steps checkup, a provider may complete the Texas Health Steps checkup and complete the documentation for the sports physical.
