Updated Fax Numbers for LTSS Service Coordination

Superior HealthPlan has recently consolidated its Long-Term Service and Supports (LTSS) Service Coordination regional fax lines for providers that serve STAR Kids, STAR+PLUS and STAR+PLUS Medicare-Medicaid Plan (MMP) members. All other LTSS regional fax lines will be discontinued.
- Effective August 1, 2017 - All STAR Kids regional fax lines were discontinued.
- Effective November 1, 2017 - All STAR+PLUS and MMP regional fax lines will be discontinued.
Please use the statewide fax numbers listed below for member eligibility and service-specific LTSS Service Coordination.

- All prior authorization requests should still be faxed to Medical Management at 1-800-690-7030.
